Crown deterioration. That cement slope around the flue opening cracks over time. Water infiltrates the cracks, freezes, expands, makes bigger cracks. Eventually water is running down inside your chimney structure.

Mortar breakdown. The joints between chimney bricks erode over decades. When mortar gets weak enough, water penetrates right through the chimney walls.
